Student Athlete Physical Evaluation Form

All students wishing to participate in soccer at Norman North are required to complete a physical evaluation form filled out and signed by a Physician.

You cannot fill out the physical evaluation form online.

You must print this form and take it with you to the physician.

Once your physical evaluation has been completed, you must scan the entire completed form (or take a photo) and upload the physical using the following link:

Your physical evaluation form will not be accepted in person or by email, only by upload.
